Perry County Memorial Hospital offers a hospital based home health program that provides a smooth transition from hospital to home with the assurance of on- going quality care. It's a team effort involving the patient, doctor, family and our Home Health Staff and is available part- time to all homebound patients including the disabled, chronically ill and those recovering from surgery. Home Health staffers are on- call 24 hours a day.
