We provide comprehensive medical care to adults of all ages for chronic and acute illnesses with a focus on preventative care. Access to care is available through walk-ins, scheduled appointments and through open access scheduling. We also provide assistance with Social Services, Flu and Pneumonia vaccines, on-site Lab services and transportation for ECHO related appointments. New patients are welcome and being accepted at all sites. We have referral arrangements for specialists for our established patients with several offering sliding scale arrangements. It is our organizational policy to not prescribe narcotics and tranquilizer’s for long term use nor do we provide long term pain management. We provide medical interpreter services for Spanish and have a language line for other languages. We have after hours coverage for established patient calls through our ECHO provider teams.
ECHO Community Health Care, Incorporated, is a proud collaborator with Deaconess Health System, St. Mary’s Medical Center, United Way of Southwestern Indiana and the Welborn Baptist Foundation. In late 2011, these organizations joined forces to facilitate a community wide ‘needs assessment’. The Community Health Needs Assessment (CHNA) was conducted as a requirement of the Affordable Care Act (ACA) for some organizations. Additionally, it serves as an evaluation tool regarding the overall health of our community.
